For most of medical history, the gut and the brain were considered separate systems — the gut digested food, the brain processed thought, and the two communicated only in the most basic ways. That understanding has been fundamentally revised over the past two decades.
We now know that the gut and brain are in constant, bidirectional communication through what researchers call the gut-brain axis — a complex network of neural, hormonal, and immune signals that connects your digestive system to your central nervous system. What happens in your gut does not stay in your gut. It affects your mood, your cognition, your energy, your immune function, and your metabolism in ways that are only beginning to be fully understood.
The enteric nervous system — the network of neurons lining your digestive tract — contains approximately 500 million nerve cells. It communicates with the brain primarily through the vagus nerve, which runs from the brainstem to the abdomen and carries information in both directions.
Approximately 90 to 95 percent of the body's serotonin is produced in the gut, not the brain. Serotonin is a neurotransmitter that regulates mood, appetite, sleep, and cognitive function. When gut health is compromised, serotonin production and signaling are disrupted — which is one reason gut dysfunction so frequently co-occurs with depression, anxiety, and cognitive impairment.
The gut also houses approximately 70 percent of the immune system. The intestinal lining is a critical barrier between the contents of the gut — including bacteria, food particles, and metabolic byproducts — and the bloodstream. When this barrier is compromised, it sets off a cascade of immune activation and systemic inflammation that affects virtually every system in the body.
The intestinal lining is a single layer of cells held together by tight junction proteins. These proteins act as gatekeepers, allowing nutrients to pass into the bloodstream while keeping bacteria, undigested food particles, and toxins out.
When tight junctions are disrupted — by chronic stress, poor diet, alcohol, medications like NSAIDs, infections, or dysbiosis — the intestinal barrier becomes more permeable. Substances that should remain in the gut begin to leak into the bloodstream, triggering an immune response. This is what is commonly referred to as "leaky gut," or more precisely, increased intestinal permeability.
The resulting systemic inflammation does not stay localized. It affects the brain (contributing to neuroinflammation, brain fog, and mood disorders), the joints (contributing to pain and autoimmune conditions), the thyroid (contributing to Hashimoto's thyroiditis), the skin (contributing to eczema and acne), and metabolic function (contributing to insulin resistance and weight gain).
The gut microbiome — the approximately 38 trillion bacteria, fungi, and other microorganisms living in your digestive tract — plays a central role in the gut-brain connection. A diverse, balanced microbiome produces neurotransmitters, short-chain fatty acids, and other metabolites that support brain function, regulate inflammation, and influence mood.
Dysbiosis — an imbalance in the microbiome characterized by reduced diversity and overgrowth of pathogenic species — is associated with depression, anxiety, cognitive impairment, and fatigue. Research has shown that the composition of the microbiome differs significantly between people with and without depression, and that interventions targeting the microbiome can improve mood outcomes.
This does not mean that depression is simply a gut problem. But it does mean that gut health is a meaningful contributor to mental health — and one that is rarely evaluated in conventional psychiatric care.
The microbiome also plays a significant role in metabolism and weight regulation. Certain bacterial species are more efficient at extracting calories from food. Others produce short-chain fatty acids that improve insulin sensitivity and reduce inflammation. The composition of your microbiome influences how your body processes and stores energy.
Dysbiosis is associated with increased intestinal permeability, systemic inflammation, and insulin resistance — all of which promote fat storage and make weight loss more difficult. Patients who struggle to lose weight despite careful diet and exercise often have significant gut dysfunction that is not being addressed.
Gut dysfunction does not always present as obvious digestive symptoms. Many patients with significant intestinal permeability and dysbiosis have minimal or no gastrointestinal complaints. Instead, they present with:
