Why Your Doctor Says Your Labs Are Normal But You Still Feel Terrible

You have been to your doctor. You have had blood drawn. You have waited for results. And then you have heard those four words that somehow make everything worse: "Your labs look normal."

But you do not feel normal. You are exhausted before noon. Your brain feels like it is wrapped in cotton. You have gained weight despite eating carefully. You cannot sleep through the night. And yet, according to the paperwork, everything is fine.

You are not imagining it. And you are not alone. This is one of the most common experiences that brings patients to functional medicine — and there is a precise biological reason it keeps happening.

The Problem With "Normal"

Standard lab reference ranges are built from population statistics, not from what is optimal for human function. When a lab says your thyroid-stimulating hormone (TSH) is "normal," it means your result falls somewhere within the range of values collected from a large sample of people — many of whom may themselves be symptomatic, medicated, or metabolically compromised.

The conventional TSH reference range, for example, is typically 0.5 to 4.5 mIU/L. Functional medicine practitioners use a much tighter optimal range of approximately 1.0 to 2.0 mIU/L. A TSH of 4.2 is technically "normal" — but for many people, it is associated with significant fatigue, brain fog, cold intolerance, and weight gain. The number is inside the reference range. The person is not functioning well.

This gap between "normal" and "optimal" is where millions of patients fall through the cracks of conventional medicine.

What Standard Panels Typically Miss

A routine annual physical usually includes a basic metabolic panel, a complete blood count, and perhaps a lipid panel. These are useful screening tools. But they are not designed to answer the question: Why does this person feel this way?

Here are some of the most commonly missed drivers of chronic symptoms that do not appear on a standard panel:

  • Free T3 and Reverse T3 — Standard thyroid panels only measure TSH and sometimes T4. Free T3 is the active thyroid hormone your cells actually use. Reverse T3 is an inactive form that can block T3 receptors. Neither is routinely ordered.
  • Fasting insulin — Insulin resistance can be present for years before blood sugar becomes abnormal. Fasting insulin reveals early metabolic dysfunction that a standard glucose test will miss entirely.
  • High-sensitivity CRP and homocysteine — Markers of systemic inflammation and cardiovascular risk that are rarely included in routine panels.
  • Ferritin — Low ferritin causes profound fatigue even when hemoglobin is normal. Many patients are told they are not anemic when their iron stores are critically depleted.
  • 25-OH Vitamin D — Vitamin D deficiency is associated with fatigue, depression, immune dysfunction, and musculoskeletal pain. It is rarely tested unless specifically requested.
  • Sex hormones — Estradiol, progesterone, testosterone, DHEA-S, and SHBG are almost never included in a standard panel, yet hormonal imbalance is one of the most common drivers of fatigue, mood changes, weight gain, and poor sleep in both men and women.
  • Cortisol patterns — A single morning cortisol measurement tells you almost nothing about how your stress response is functioning throughout the day. A four-point salivary cortisol test maps the full diurnal pattern.

Why Doctors Do Not Order These Tests

This is not a criticism of your doctor. Conventional medicine operates within a system that is optimized for diagnosing and treating acute illness. A 15-minute appointment is designed to rule out serious pathology — not to investigate why a generally healthy person feels chronically unwell.

Insurance reimbursement structures also play a role. Many of the tests that functional medicine practitioners find most useful are not covered by standard insurance plans, which creates a financial disincentive to order them in a conventional setting.

The result is a large population of people who are genuinely suffering — but whose suffering does not register on the instruments conventional medicine uses to measure it.

What Functional Medicine Does Differently
