Optivita Health operates an SMS/text messaging program to communicate with patients and prospective patients. By providing your phone number and opting in to receive SMS messages from us, you agree to this Policy and our Terms of Service.
We collect your mobile phone number when you voluntarily provide it through our website forms, patient intake forms, or by texting a keyword to our SMS number. We may also collect information about your interactions with our SMS messages, such as whether you have opened or responded to a message.
We use your mobile phone number solely to send you SMS messages related to: appointment reminders and confirmations, health information and educational content, clinic updates and announcements, follow-up care information, and responses to your inquiries. We will not use your phone number for any other purpose without your explicit consent.
By providing your mobile phone number and agreeing to receive SMS messages from Optivita Health, you expressly consent to receive text messages from us. Your consent is not a condition of purchasing any goods or services. Message frequency varies. Message and data rates may apply.
You may opt-out of receiving SMS messages at any time by replying STOP to any message you receive from us. After you send the STOP message, we will send you a final message confirming that you have been unsubscribed. You will not receive any additional SMS messages from us unless you re-opt in. You can also contact us directly at care@optivitahealth.com or call our office to request removal from our SMS program.

We will not sell, rent, or share your mobile phone number or any personal information collected through our SMS program with third parties for their marketing purposes. We implement reasonable technical and organizational measures to protect your personal information from unauthorized access, use, or disclosure. However, no method of transmission over the internet or electronic storage is 100% secure.
As a healthcare provider, Optivita Health is committed to protecting your health information in accordance with HIPAA. SMS messages sent through our program may include general health information but will not include protected health information (PHI) without your prior written authorization. Any PHI shared with us remains subject to our HIPAA Notice of Privacy Practices.
Message frequency varies based on your interactions with our clinic and your care needs. Message and data rates may apply depending on your mobile carrier and plan. Please check with your carrier for details about your specific messaging rates.
SMS messaging is provided as a convenience and is not a substitute for medical advice, emergency services, or direct communication with your healthcare provider. In case of a medical emergency, please call 911 or go to your nearest emergency room. Optivita Health is not liable for any delays or failures in SMS delivery due to technical issues beyond our control.
This Policy is governed by the laws of the State of Oregon. Any disputes arising under this Policy shall be subject to the exclusive jurisdiction of the courts located in Washington County, Oregon.
